Thank you very much for your interest in using the University of Liverpool School of Medicine EDI Curriculum Toolkit. So that we can understand its use and develop it further, we’d be grateful if you can answer a few questions about the way you have used or intend to use the toolkit.

Please note there is no way for us to identify individuals from the data collected here, and once submitted, there is therefore no way for us to be able to remove your responses from the evaluation. Please therefore only proceed to the questions if you are happy to do so.  Taking part in this evaluation is voluntary and will not impact upon your ability to access or use the toolkit in any way.

If you do complete the evaluation, the answers you give will be downloaded and stored on a secure drive at the University of Liverpool.  The responses will be used to shape future developments of the toolkit, and potentially as part of presentations and publications related to the evaluation of its development.  Data will be available to the Curriculum Toolkit project team. Data will be kept for ten years, in line with good research practice, at which point it will be deleted from the University system.
